How to Apply:
       
        1. Buy P15,000.00 worth of purchase or accumulate P15,000.00 worth of Hush Puppies merchandise with in the  
        year 2011 and sign up application form at any Hush Puppies boutique to enjoy  2 years of membership privileges
        (from date of issue).

        2. Simply fill in the application form with full details and submit together with the accumulated receipt at any Hush 
        Puppies boutique.

Renewal:

        1. HUSH PUPPIES VIP CARD must be renewed  before the card expires. Renewal can be done at any Hush 
        Puppies boutique.

        2. HUSH PUPPIES VIP CARD can be renewed by accumulating P10,000 worth of Hush Puppies Merchandise 
        in receipts

        3. Renewal of the HUSH PUPPIES VIP CARD follows the same process as its application.

Terms & Conditions:

        1. The HUSH PUPPIES VIP CARD has to be presented to the cashier at the time of purchase.

        2. HUSH PUPPIES VIP CARD is valid at Hush Puppies boutique only, not valid in department stores or other 
        retailers. Ask the Hush Puppies Sales Associate for an updated store list.

        3. HUSH PUPPIES VIP CARD is valid for a period of two (2) years.

        4. Discount privileges, terms & conditions may be amended or withdrawn without prior notice at the sole discretion
        of Hush Puppies.

        5. A member who fails to renew his/her card upon expiry will considered a new member in his/her subsequent  
        application.

Non-transferable:
   
        The HUSH PUPPIES VIP CARD is non-transferable and Hush Puppies reserve the right to verify the identity 
        of the cardholder whenever it is used.

Lost/Damaged or theft of card:
   
        In the event that the HUSH PUPPIES VIP CARD is lost, stolen or damaged, please notify our Hush Puppies 
        boutique personnel.

Processing Time

        Please allow 1-2 weeks for the processing of your HUSH PUPPIES VIP CARD.
